Seeing a new doctor can be an overwhelming experience. There is a lot to worry about. Am I going to have enough time with the doctor? Will he have enough time to answer all my questions? Will he be able to help me?

At The Acupuncture & Chiropractic Center it is our intention to set enough time aside for all new patients so that they can have all their questions answered. The doctor will go over all of your paperwork, both current and past medical histories with you. He will then talk with you to find out what your primary goals are. Dr. Cowan will then determine which tools to use that will give you the quickest and long lasting relief for your condition.

Before you visit our office you may fill out all the necessary paperwork. You can mail, email, or fax the paperwork to our office. This cuts down on the time spent waiting.

Please choose the form in which you are seeking treatment. Please fill out the forms as complete as possible. You may not think some of this information is not important to Dr. Cowan, but it is. He wants to be able to assess your condition from all sides.
